Halton Food Connect is a collaborative initiative between Halton Region and three community agencies: Burlington Food Bank, Food For Life, and Kerr Street Mission. The program’s goal is to help improve access to healthy food for individuals who are qualified recipients through the Ontario Works program. The program is based on a once-a-month delivery to households providing healthy food deliveries.

 

As an Intake Coordinator, you will work in tandem with other team members to collect/organize/verify client information and interact with clients on an ongoing basis to provide program support. You will work with a registration platform and will be responsible for the accurate capturing of all necessary data in the intake process. In addition, your roles and responsibilities will include but not be limited to:

Ø  Communicating with clients by phone and email; confirming delivery schedules and following up on satisfaction

Ø  Educating and enabling clients to other available food security programs available to them in their community

Ø  Collecting and communicating client feedback to the Program Manager

Ø  Sharing community resources and referrals

 

Your day-to-day reporting will be to the Program Manager of Halton Food Connect via the Burlington Food Bank and may involve working collaboration with all stakeholders.
